Project duration: About 2 years
Project goal: Improve depression care, provide strategic development support and develop an on-boarding curriculum for an FQHCs Primary Care Behavioral Health (PCBH) program.
Several CFHA experts contributed to this project which was flexibly developed over a period of two years. The project started with coaching for the leaders of the PCBH program and then focused on the development of depression care pathways in primary care following the Collaborative Care model (CoCM). CFHA also customized a set of slide decks for the clinic to use to orient new behavioral health staff in their on-boarding process.
