Care Manager Skillset Immersion Webinar

The Care Manager Skillset Immersion Webinar, presented by Daniela Vela Hernandez, is meticulously crafted for Behavioral Health Care Coordinators (BHCCs) and those transitioning into the pivotal Care Manager role within the CoCM program.   • $49 for trainees and non-licensed care team members (CFHA Members)

Meet Daniela Vela Hernandez, a member of our Technical Assistance team. With a robust background in clinical work across multiple settings, Daniela focuses on supporting organizations with the implementation and optimization of their behavioral health integration programs.

Daniela’s expertise lies in the Collaborative Care Model (CoCM).  She has gained extensive experience in this model, whether its preparing primary care providers Behavioral Health Care Managers, Daniela’s guidance has been instrumental in enhancing training and implementation of this model.

Approachable, empathetic, and knowledgeable, Daniela simplifies complex concepts into clear, understandable language. Trust her to guide you through the nuances of integrated care with a conversational tone, expert knowledge, and a commitment to educating and connecting with her audience.
